Aged and Disability

  
The Aged and Disability Unit (A&D) core responsibility of the unit is to provide services to aged people and younger people with disabilities living in the City of Wanneroo community who, in the absence of basic maintenance and support services are at risk of premature or inappropriate long-term residential care.

Home and Community Care (HACC) Program

Domestic Assistance  

Refers to assistance with domestic chores, including assistance with home cleaning, shopping (unaccompanied)

Home Delivery Meal Service

Refers to those meals which are prepared and delivered to the clients home. It does not include meals prepared in the client's home

Personal Care

Refers to assistance with daily self-care tasks, such as bathing, dressing, grooming, getting in and out of bed.

Respite

(Carers) provided by the HACC Program to carers in order to give them relief from their caring role, including advocacy and the provision of advice, information

Delivered Meals

Refers to a subsidised home delivered meal service, eligible people can order up to 7 meals per week

Social Support

Refers to assistance provided by a companion (paid worker or volunteer), either within the home environment or  accessing community services.

Social Support Group Activities

Refers to assistance provided by a paid worker or volunteer for socially isolated people to join together within a group for social activities/excursions.

Shoppers Bus Transport

Refers to shopper bus transport assistance, the Aged and Disability Service unit provides transportation to isolated clients to and from major shopping centres each fortnight.

Allied Health Care

Refers to professional allied health care services, the Aged and Disability Service unit provides specialist services in podiatry.

Centre Based Day Centres

The City of Wanneroo operates six (6) Centre Based Day Care Centres at the following locations within the City: -

  • Alexander Heights Community Care Centre, 18 Linto Way, Alexander Heights
  • Offers Recreational Programs to : - Vietnamese Group Italian Group Dementia Specific Mainstream (Frail and Elderly) Group
  • The Heights Community House, 18 Linto Way, Alexander Heights
  • Offers Recreational Programs to Younger People with Disability. 
  • Aboriginal Seniors Group (Kookaburra Club), Denston Way, Girrawheen
  • Offers Aboriginal Seniors the opportunity to meet and socialise. 
  • Wanneroo Adult Day Centre, Wanneroo Recreation Centre, Scenic Drive, Wanneroo
  • Offers Recreational Programs to Mainstream (Frail and Elderly) 
  • Quinns Rocks Adult Day Centre, Gumblossom Community Centre, Tapping Way, Quinns Rocks
  • Offers Recreational Programs to: - Mainstream Group (Frail and Elderly) 40 0s-60 0s Group Men 0s Group (Frail and Elderly) Dementia Specific Group Younger People with Disability Younger women with Disability
  • Yanchep/Two Rocks Adult Day Centre, Phil Renkin Community Centre, Two Rocks Offers Recreational Programs to Mainstream Group (Frail and Elderly)

The above services are provided to the functionally disabled frail aged and younger people with disability, there is a fee for service that may be reduced or waived if there is financial hardship.

Community Aged Care Packages (CACP)
Individually planned and coordinated packages of care tailored to help older Australians remain living in their own homes. They are funded by the Australian Government to support the complex care needs of older people. The Aged and Disability Unit currently provides CACP to 18 clients throughout the City of Wanneroo.

The above service is provided to the frail aged with complex care needs, there is a fee for service that may be reduced or waived if there is financial hardship.
