IMPORTANT INFORMATION

This form is for:

• Any recurring/regular booking (up to a maximum of 10 weeks – see bulk booking cycles below)
• Any casual booking where the cost of the booking in any one day totals more than $200
• Court bookings only (excludes room hire or other)
• Bulk booking fees are set in accordance with the City’s Schedule of Fees and Charges and are payable in full no later than 7 days prior to commencement of the first booking.
