This report is to summarise a Contractors’ performance in their service delivery for the duration of the contract and, if applicable, assist in decision making for extensions and/or project works under the terms of the agreement. The report may also assist tender evaluation panels in scoring previous experience and capability for future projects (for similar works). This report is for internal purposes only. Please consult with internal stakeholders for assistance completing this report.

Rating scale
| 1 - Poor | Marginally adequate, does not satisfy all requirements. Successful completion uncertain. Concerns regarding competence or capacity and ability to successfully fulfil the contract requirements. |
| 2 - Needs improvement | Acceptable and mainly compliant, generally meets the requirements except for minor aspects and shortcomings. Successful completion likely. |
| 3 - Acceptable | Satisfies all requirements. Acceptable and compliant. Successful completion highly probable. No short comings apparent. |
| 4 - Very good | Negligible risk of failure. Satisfies the requirements in all respects. Highly competent. |
| 5 - Excellent | Superior. Represents industry best practice. Fulfilment of the requirements at the highest possible level. |
