Use this form to:

• Request an extension for payment
• Have your infringement reviewed

To have an infringement reviewed, you will need to provide the following information:

• Infringement number
• Full name, postal address, email address & contact number
• Copies of all relevant supporting documentation
• Detailed reasons why you believe the infringement is to be reviewed

If you are requesting that the infringement be reviewed on any of following grounds, you will need to upload the relevant supporting documentation:

Nominate a driver

• If you were not the driver or person responsible for the vehicle at the time of the alleged offence, you are required to provide the driver's full name and address to the City of Wanneroo within 28 days of the infringement being issued. 

Should the driver's full name and address not be provided, legislatitively you will be deemed to have committed the offence.

Disabled parking permit not displayed

• Upload both sides of your current disabled parking permit.

Vehicle breakdown or accident

• Written evidence must be provided from the RAC, reputable mechanical firm or attending tow truck driver who rectified the fault confirming that the vehicle could not be moved.

Medical emergency

• Written evidence must be provided from the medical centre, practitioner’s surgery or the hospital verifying the medical emergency at the time of the alleged offence.

If you do not have written evidence, please provide a signed and witnessed statutory declaration verifying that circumstances at the time of the alleged offence.  You must upload the statutory declaration once it has been completed, signed and witnessed.
