By signing the Media Consent and Release Form or Media Consent and Release (Bulk Collection) Form, you acknowledge and agree that the following statements are true and correct:

1. YOU HAVE BEEN GIVEN THE OPPORTUNITY to:
   1. read this document, Media Consent and Release Form or Media Consent and Release (Bulk Collection) Form and the Privacy Policy and understand the content described within;
   2. communicate and raise questions with a representative of the City about the way the Personal Information may be handled by the City, and you are satisfied with the answers received;
   3. limit the scope of your consent or use a pseudonym by providing details of such limitation or pseudonym in writing to the City’s Communications and Brand team at media.enquiries@wanneroo.wa.gov.au; and
   4. refuse to provide your consent to the collection of your Personal Information for use as Published Content by the City.
2. YOU UNDERSTAND that:
   1. your provision of Personal Information is entirely voluntary;
   2. there is no guarantee that your Personal Information collected by the City will be used in Published Content;
   3. neither you, your child nor the individual for whom you are the legally appointed decision maker will be entitled to any fee or compensation from the City for the use of Personal Information in Published Content;
   4. images or recordings of the Personal Information collected may be subject to reasonable retouching or alteration, and/or quotes or testimonials may be extracted or cut-down;
   5. Personal Information published on the internet is accessible to anyone in the world with access to the internet and, once information has been published on the internet, the City has no control over its subsequent use and disclosure; and
   6. where you have limited or subsequently withdrawn your consent by written notice to the City's Privacy Officer, such limitation or withdrawal will only apply from the date the City receives your notice to limit or withdrawn your consent and any prior use and disclosure of your Personal Information (in accordance with your consent previously given) will not be able to be limited or withdrawn.
3. YOU WARRANT that:
   1. where the information you provide the City includes intellectual property owned by a third party, you have permission from the copyright owners to collect, use and disclose, and to allow the City to collect, use and disclose, that information; and
   2. to the best of your knowledge there is no indication that the City’s collection, use and disclosure of a child’s Personal Information is contrary to the child’s best interests.
4. YOU ASSIGN to the City the ownership of any copyright you own or may acquire in the Published Content to the extent necessary to allow the City to exercise its rights without restriction.
5. YOU WAIVE any rights, claims or interests you, your child or the individual for whom you are the legally appointed decision maker may have to control the use of the Personal Information collected by the City.
6. YOU IRREVOCABLY RELEASE the City in respect of any right, claim or interest that you, your child or the individual for whom you are the legally appointed decision maker may have arising out of the collection, use and/or disclosure of the Personal Information in Published Content, to the extent permitted by law.
