Online forms: Donation Application Form

Before completing this application you must ensure you have carried out the following (if applicable) and have the documentation ready to upload as part of your application. 

The maximum upload size is 50MB per page.  

You will be required to provide the following documentation to support your application:

• Copy of certificate of insurance currency
• If you do not have an ABN, you need to complete and upload a Statement by supplier not quoting an ABN form
• If the location of your venue is a City of Wanneroo facility, you need to attach booking information
• If this is an event held in the City of Wanneroo, please complete and submit a Public Community Event Notification Form and upload the receipt

Note: This form will timeout after 30 minutes of inactivity per page.
